Wladyslaw Kozak, 82 of Harwood Heights, IL passed away peacefully Sunday February 14th 2021 at home surrounded by his family. Born September 23rd 1938 in Bratucice, Poland to Zofia and Andrzej Kozak.
Wladyslaw is survived by his wife Stanislawa of 49 years marriage. He was father to two children Bernadette(Mark) and Paul(Ula). He was a devoted grandfather to 4 amazing grandchildren Joshua, Maria, Brian and Julianna. In lieu of flowers, Memorial contributions can be made to American Heart Association.
